Job Description

We are seeking a passionate and dedicated Aftercare Support Worker to join our team as part of the Culture Care Connect program. This role is crucial in providing culturally appropriate aftercare support to Aboriginal and Torres Strait Islander individuals who have experienced a suicidal crisis. Working closely with a supportive team, you will deliver short- to medium-term interventions that promote healing and connection, ensuring individuals receive timely, comprehensive care.

Key Responsibilities:

  • Provide aftercare services within 24 hours of referral for individuals following a suicide attempt or crisis.
  • Engage in ongoing risk assessments and safety planning using a strength-based approach.
  • Support clients in attending medical and social support appointments.
  • Develop care plans and collaborate with community and health services for ongoing care.
  • Promote Social Emotional and Wellbeing (SEWB) services and participate in community events.
  • Maintain accurate client records and ensure data collection for continuous improvement.

Desired Skills and Experience

About You:
To succeed in this role, you will need:

  • Essential:

    • Aboriginality is an essential criterion under Section 50(d) of the Equal Opportunity Act 1984 (WA).
    • Certificate IV in Mental Health or Aboriginal and/or Torres Strait Islander Primary Health Care (or equivalent).
    • Demonstrated experience working with clients experiencing Social Emotional Wellbeing (SEWB) challenges, particularly in Aboriginal communities.
    • Strong interpersonal skills and the ability to establish relationships with community members and service providers.
    • Current WA driver’s license and the ability to travel for outreach services.
    • Current Working with Children Check and National Police Clearance (or ability to obtain).
  • Desirable:

    • Experience in suicide prevention services and/or SEWB programs.
    • Training in SafeTALK, ASIST, or other relevant suicide prevention programs.
